Pandemic: On the Brink inclui novas cartas de evento, novos papéis, regras para cinco jogadores, e um jogo opcional com desafios para aumentar a dificuldade do jogo base Pandemic. Estes novos desafios, que podem ser usados individualmente ou em combinação de modo a tornar o jogo ainda mais difícil, estão a seguir: -O Virulent Strain que torna uma doença particularmente mortal de maneiras imprevisíveis. O desafio Mutation acrescenta uma quinta doença (roxa), que se comporta de forma diferente das quatro originais. O desafio Bio-Terrorist coloca um jogador contra os outros.
